How do I fill out the Request certificate?
The following fields are mandatory:
- Personal Details
- Conformity box
- Name
- NIF or NIE
- Date of birth
- Country of nationality
- Father’s given name (You must fill in this field as it appears on your ID)
- Mother’s given name (You must fill in this field as it appears on your ID)
- Country of birth
- Province of birth
- Place of birth
- Contact Information
- Mobile Phone Always include the country code (+34 Spain). For example: +34600606060.
- Purpose
- Purpose for which the certificate will be used
